Understanding Shoulder Pain

The shoulder is the most mobile joint in your body, which is exactly why it is prone to injury. What is causing Shoulder Pain?

Rotator Cuff Tendinopathy & Tears

The rotator cuff is a group of four muscles (supraspinatus, infraspinatus, teres minor and subscapularis) and tendons that stabilise the shoulder joint and allow you to lift and rotate your arm. It's the most frequent source of shoulder pain, particularly from overuse, repetitive overhead activity, or gradual wear and tear or deconditioning with age. Tears can range from small, partial tears to complete ruptures — and, perhaps surprisingly, some rotator cuff tears cause no pain at all and are only discovered incidentally on imaging.

Subacromial Impingement

This occurs when the rotator cuff tendons become compressed as they pass through a narrow space beneath the top of the shoulder blade, particularly during overhead movement. It's a common cause of pain when reaching up, out to the side, or behind the back.

Frozen Shoulder

Frozen shoulder involves progressive stiffening, pain, and a significant loss of movement in the shoulder joint, often developing gradually over months. It has a lifetime prevalence of around 2–5% in the general population and is notably more common in people with diabetes.

Shoulder Bursitis

Inflammation of the small fluid-filled sac (the bursa) that cushions the shoulder joint, often occurring alongside rotator cuff irritation or impingement, and typically felt as pain with overhead or reaching movements.

Shoulder Osteoarthritis

Degenerative wear of the cartilage within the shoulder joint, more common with age, usually producing a deep ache that worsens with activity and eases with rest.

Referred Pain from the Neck

Not all shoulder pain originates in the shoulder itself. Problems in the cervical spine, including disc issues or nerve irritation, can refer pain into the shoulder and upper arm, which is why a thorough assessment often includes the neck as well.

AC Joint Injuries

Sprains or arthritis affecting the acromioclavicular (AC) joint — where the collarbone meets the shoulder blade — are common after falls onto the shoulder or direct trauma, such as in contact sports.
